Case Management
Case managers are nurses, social workers or other health care professionals. They can help you learn more about health conditions you may have. They also work with your Primary Care Provider (PCP) when medical questions come up. They help with communication between your PCP, specialists and caregivers. They can help you get rides to appointments. They can help you get special equipment and home health services if you need them.
Aetna Better Health members do not need a referral for case management. As an enrollee in the Illinois Integrated Care Program, you can get this service at no cost to you. Call Member Services toll free at 1-866-212-2851 and ask how you can sign up for case management through our care management program.
