Home and community-based care will be provided during Service Packages II and III. This service option is for members with a disability who live in the community and who need support to perform their activities of daily living. Aetna Better Health provides a case manager who will complete an evaluation in the member's home or place of residence. This case manager and member (or their representative) will then develop a plan and identify services to keep the member as independent as possible. Examples of the types of services that can be provided include personal care, home health care, assisted living and adult day care.
