We would love for you to be our member! We have designed our Managed Long Term Care program to help you with your special health care needs. We know it’s important to you to live where you want. So we help you do that. We want you to get the quality care you deserve.
Our care starts with a care manager who:
- Meets with you to review your needs
- Puts together a care program just for you
- Supports your family and caregivers
- Helps you get health screenings and vaccinations
- Provides support for your chronic illnesses, such as diabetes or asthma
- Coordinates home- and community-based services
