Frequently Asked CHIP Questions
How do I get a CHIP application?
There are five ways to apply for CHIP:
- > Complete an application online by going to the state's COMPASS website. Keep in mind you will need to send in your income documents to complete the application.
- > Download an application from the link below to print an application. This will require you to have Adobe Acrobat Reader installed on your computer to open and print file. Upon completion, please send the application with income documents to the address on the application or fax to 1-860-754-1055.
- > CHIP Application Forms English/EspaƱol
- > Request a paper application be sent to you by emailing a request to chipofpa@aetna.com. Include your name and mailing address in your e-mail.
- > Apply over the phone by calling 1-800-986-KIDS (5437).
Help with your application is available Monday through Friday, from 8 a.m. to 5 p.m.

- > You can download and print the application in a PDF format. The application must be completed and mailed to us along with the income verification and the signature page in order to process the application.
- > You can call 1-800-822- 2447 and ask that an application be sent to you.
How do I complete the application?

- > Be sure to answer all questions. An incomplete application will delay processing.
- > Include appropriate income documentation. An application that is submitted without income documentation is considered incomplete.
