If you or your child had an acute hospital stay you may receive a call from our Care Management Department. An Aetna Better Health of Michigan staff member will ask you questions related to your hospital stay to help with:

• Scheduling follow up appointments with PCP or specialist

• Assist with obtaining prescribed medications

• Help you to understand your discharge instructions

• Assist with any additional needs
