How do you earn rewards?
You can earn rewards for completing preventive care. This is the best way to keep you and your family healthy. Everyone needs regular checkups and health screenings. They can give you peace of mind that you’re as healthy as you feel. They can also help your provider find any health problems early, when they’re easier to treat.
There is no extra cost for preventive care. Better yet, we reward you for taking steps toward health.
Earn gift cards
You can earn gift cards for each healthy step you complete. So it really does pay to check on your health.
Screenings
$25: Get your mammogram every year (ages 54 to 74) to screen for breast cancer
$25: Get your cervical cancer screening
$25: Complete a diabetes A1C test
$35: Get a screening for chlamydia
Wellness visits
$25: Complete your wellness checkup(s) (birth to age 12 years)
$35: Complete your wellness checkup (ages 13 to 19 years)
Vaccines
$25: Get your COVID-19 vaccine
$25: Get your flu shot once a year
$35: Complete all immunizations (birth to age 2 years)
$25: Get your HPV (human papillomavirus) vaccine
Pregnancy and birth care
$75: Go to your first prenatal visit with your provider (within the first trimester and within 42 days of letting us know you are pregnant)

Up to $75: Go to your pre- and postnatal visits with your provider

How to get your gift card
How to get your gift card
Your provider will submit a claim for your completed screening, annual visit or care. A claim is a form your provider sends us. That’s how we know you went for a visit. Once we receive the claim, we’ll send your gift card letter in the mail. Look for your gift claim code and instructions in that letter.
Pregnancy and birth care rewards
If you’re pregnant or have a newborn and want to report your visits to earn a gift card, you’ll have a different process. Your care manager will send you a form to track your visits. Just take the form with you and have your provider fill it out at each appointment. Then send your form back to your care manager once it’s complete. Your gift card will be mailed to you.
How to use your gift card
You can choose from a variety of stores and restaurants in our online gift card store. Just remember, you won’t be able to use gift cards for products that can cause health risks, like alcohol, nicotine products or firearms.
