How do you earn rewards?
You can earn points toward rewards for completing preventive care and healthy actions. This is the best way to keep you and your family healthy. Everyone needs regular checkups and health screenings. They can give you peace of mind that you’re as healthy as you feel. They can also help your provider find any health problems early, when they’re easier to treat.
There is no extra cost for preventive care. Better yet, we reward you for taking steps toward health.
Earn your points
For each healthy action you complete, you can earn points. Then, you can spend your points on fun health items from the Aetna Better Care® Rewards catalog. So it really does pay to check on your health.

Screenings
Health risk survey
250 points ($25 value)
- Complete your health risk survey within 90 days of enrollment (new members, ages 19 to 64)
Breast cancer
500 points ($50 value)
- Get your mammogram to screen for breast cancer (ages 50 to 74)
Cervical cancer
500 points ($50 value)
- Complete a pap test (ages 21 to 64)
- Complete a human papillomavirus (HPV) test (ages 30 to 64 and high risk)
- Complete a pap and HPV combined test (ages 30 to 64 and high risk)
Diabetes
250 points ($25 value): Complete a qualified kidney health evaluation (ages 18 to 85 with a diagnosis of type 1 or type 2 diabetes)
250 points ($25 value): Complete an HbA1c test and a diabetes eye exam (ages 18 to 75 with a diagnosis of type 1 or type 2 diabetes)
Welcome gift: The DiabetesSmart program provides members with resources, tools, health coaching and other support to help them improve their health. Join DiabetesSmart and receive a welcome gift just for joining (members with risk factors for diabetes and those who’ve been diagnosed with type 1 or type 2 diabetes)
Lead screening
250 points ($25 value)
- Complete a blood test for lead poisoning by your child’s second birthday (ages 0 to 2 years)
Wellness visits
Adult wellness visits
500 points ($50 value): Complete a checkup with your primary care provider (PCP) within 60 days of enrollment (new members, ages 20 and up)
500 points ($50 value): Complete a checkup with your PCP (existing members, ages 20 and up)
Baby wellness visits
250 points ($25 value): Complete 3 wellness visits in the first 6 months of age (ages 0 to 6 months)
250 points ($25 value): Complete 3 wellness visits between 6 and 15 months of age (ages 6 to 15 months)
Child and adolescent wellness visits
- 500 points ($50 value): Complete an annual wellness visit with a PCP or Ob/Gyn (ages 3 to 21 years)
250 points ($25 value): Complete full childhood vaccination series against DTap, HiB, polio, HepB, chicken pox, PCV, MMR, HepA, rotavirus and the flu (age 0 to 2 years)
250 points ($25 value): Complete all three required adolescent immunizations by your child’s 13th birthday (ages 13 and under)
Pregnancy and birth care
500 points ($50 value): Go to a prenatal visit with your provider within the first trimester or within 42 days of enrolling with your plan (ages 18 and older)
500 points ($50 value): Go to your postpartum visit with your provider within 7 to 84 days after giving birth (ages 18 and older)
Welcome gift: The BabySmart program provides pregnancy and new parent resources, support, tools, health coaching and rewards. Join BabySmart and receive a welcome gift just for joining (all pregnant members and new parents)

If you complete the Step-Up Challenge, you’ll get your points right away. For all other healthy actions, we usually award points within 3 months after you complete them.
