Using your benefits and services is easy. You can get preventive care, emergency care, nursing home care and more. Just bring your member ID card to your appointment. Here are some pages that will help you understand and keep your benefits:

Get quality care and services close to home. We have a strong network of providers and pharmacies to choose from. And there are many ways to search for one.

Call 911 right away if you have a serious illness or injury. You can also use these resources when you need medical attention:

Access the Member Portal

Log in to your Member Portal. Don’t have access yet? Just complete this registration form (PDF) to create an account. Then, you can do so much more with your health plan. Like manage your plan benefits and health goals from anywhere. Or send us secure messages.

Using your Member Portal

 

Look for the “Contact us” link in the middle of the page. You’ll have access to: 

Member Services

You can call Member Services anytime with questions.

Replacement cards

You can get a new member ID card if you lose your old one. 

Provider changes

You can change your provider or specialist from anywhere. 

Information updates

You can update your account information anytime.

Forms

Find request forms, like the pharmacy exception request form.

You can ask for information about any health condition.

Are you looking for help finding a provider? Or finding services in your community? We’re here to help you get the answers you need. 

Access the MyActiveHealth℠ website

You deserve to live a happy, healthy life. And we can help. MyActiveHealth is a health tool that makes it easier to manage your health. You can use this tool at no cost. Just sign in to the Member Portal. Next, click "Tasks” and choose “Manage My Health.”  

Health surveys and records

You can keep track of your medical history and get healthy living tips based on your survey answers. 

Videos and podcasts

Learn more about health, your health plan and other wellness topics. 

Healthy lifestyle programs

They can help you meet all your goals, like quitting smoking, eating healthy, managing stress and more.

You can call 1-855-231-3716 to request a printed copy of the health survey. You can also get printed information on health conditions and wellness topics.

 

Get language assistance

 

 

For language services, call the number on your member ID card. Then, ask for an operator.
