We’ll coordinate your Medicare and Medicaid covered services for you if you:
- Have Medicare Parts A and B
- Have full NJ Medicaid (FamilyCare) benefits
- Are a full-time New Jersey resident

- SNP Members in New Jersey with Medicare and full Medicaid benefits are eligible to use the monthly SEP (Special Election Period) to enroll into another Fully Integrated Dual Eligible Special Needs Plan (FIDE SNP) or Highly Integrated Dual Eligible Special Needs Plan (HIDE SNP).
- This SEP can be used once per month and the effective date of the new plan will be the first of the following month.

Coverage and service area
With this plan, you must use in-network providers for service coverage. This includes an in-network primary care provider (PCP). You don’t need a referral for care from an in-network specialist. We cover emergencies and urgent care — even when they’re out of network.
Medicare is a federal program. But this plan is only for people who live in our service area. So you’ll need to continue living in one of our covered counties: Atlantic, Bergen, Burlington, Camden, Cape May, Cumberland, Essex, Gloucester, Hudson, Hunterdon, Mercer, Middlesex, Monmouth, Morris, Ocean, Passaic, Salem, Somerset, Sussex, Union and Warren counties.
