How To Renew Your Coverage
We want to make the renewal process as easy as possible so that you don't lose any of your health benefits. Based on where you reside, follow the steps below.

New York City Residents

You should receive your renewal documents in the mail directly from the New York City Human Resources Administration (NYC HRA). It will arrive with a self addressed stamped envelope. Our plan can help you to renew your Medicaid Coverage. When using any of the methods below you will want to have:
- Proof of all types of income (social security, retirement, disability, workers compensation, etc.).
- Current month banks statement for all accounts (ONLY IF APPLICABLE)
- Proof of cost for Supplemental Medical Insurance (ONLY IF APPLICABLE)
- Verification of Deposit from the Pooled Income Trust Company (ONLY IF APPLICABLE)
METHODS:
BY PHONE: Call our Member Services line at 855-456-9126 and ask to be connected to our Medicaid Recertification Department. The Medicaid Recertification specialist will be able to help you to fill out your forms and submit them directly online through a secure Web Portal to NYC HRA.
By Mail: Call our Member Services Line at 855-456-9126 and ask for assistance. Our plan can go through the application with you and provide any guidance you may need. It is advised that you send the form to our plan to submit directly to Medicaid through our online portal access. Otherwise, you can mail the completed form to NYC HRA in the envelope provided.
In Person: As a recipient of Managed Long Term Care Services all Medicaid Cases are reviewed at:

- Proof of all types of income (social security, retirement, disability, workers compensation, etc.).

- Proof of cost for Supplemental Medical Insurance (ONLY IF APPLICABLE)

Didn't Receive Your Renewal Form

The Local District will send you a form around the same time of year as when you first applied for your benefits. You may be sent a preprinted form to confirm your income and resources.
If you didn’t receive your renewal form in the mail, call our Member Services Line at 855-456-9126.
- Ask to speak to the Medicaid Recertification Department. Our plan can assist in obtaining a copy of the documents needed.

Create an online account by using the link above. There, you can print out copy of the forms. Please follow the steps in the previous pages. (You cannot renew online, you must print out the forms.)
