Kids’ health and wellness
Kids are our future. These extra benefits promote health and learning to ensure kids have the support they need to succeed. Earn rewards for completing well-child exams. Or get help paying for after-school programs.

Ages birth to 15 months
Earn $60 once per lifetime for completing six well-child exams for kids ages birth to 15 months.
Ages 16 to 30 months
Earn $40 once per lifetime for completing two well-child exams for kids between ages 16 and 30 months.
Ages 3 to 20 years
Earn $25 once each year for a well-child exam for kids ages 3 to 20.

Earn $50 for completing the full combo 3 series of childhood immunizations on or before your child’s second birthday.

Pregnant members can earn $25 for notifying their health plan of their pregnancy. To notify us about your pregnancy, join our digital app, Maven, and then complete the health assessment.

Prenatal visits
Earn $25 for completing your first prenatal visit within the first trimester. Then earn $10 for completing every prenatal visit (up to 10 visits) after your first one.
Postpartum visits
Earn $50 for your first postpartum (after birth) visit up to 21 days after birth. And then earn $50 for your second postpartum visit 22 to 84 days after birth.

Pregnant members can get a no-cost breast pump. You can get this benefit during your third trimester, once every two years. And you don’t need pre-approval or prior authorization.

Members ages 12 months and younger can get $100 to buy diapers, once per lifetime.

Pregnant members (who are 37 or more weeks pregnant) can earn a $90 gift card. You can use this gift card to help pay for a portable crib or car seat. To earn your gift card, you’ll need to complete 7 prenatal visits during pregnancy.

Breast cancer screening
Female members (ages 40 and older) can earn $25 for completing a breast cancer screening.
Colon cancer screening
Members (ages 45 and older) can earn $25 for completing a colon cancer screening.

Kidney screening
Members (ages 18 to 64) who have a diabetes diagnosis can earn $15 for completing kidney health screenings.
Diabetic retinal eye exam
Members (ages 18 to 64) who have a diabetes diagnosis can earn $15 for completing a diabetic retinal eye exam.
Diabetic A1C testing
Members (ages 18 to 64) who have a diabetes diagnosis can earn $15 for completing A1C testing.
Healthy foods
Members ages (18 to 64) who have a diabetes diagnosis and are enrolled in care management can get $50 each month for healthy food and groceries. To get this reward, you need to be enrolled in care management and complete all of the diabetes screenings above.

Get three meals each day for up to 12 weeks when you return home from an inpatient stay at a hospital.

Members who have an asthma diagnosis and are enrolled in care management can get $250 each year to manage home-based asthma triggers. You can use this benefit amount toward supplies and services, like:

Members (ages 21 and older) who have a chronic pain diagnosis and are enrolled in care management can get $500 to use toward treatment, such as:

American Indian and Alaskan Native members can get up to $400 each year for traditional healing practices. To get this benefit, you must complete your health risk screening.

Mental health and substance misuse
Mental health is a big part of your overall health. That’s why we offer several ways to help, including support for treating substance use disorders as well as paying for behavioral health visits and recovery housing. You can also get app tools that offer digital companionship.

Members (ages 6 and older) can earn $20 for each follow-up visit (up to 4 visits) with a provider within 30 days of a hospital or facility stay for behavioral health.

Members (ages 13 and older) can use Pyx Health. This app offers you 24/7 digital companionship and support to meet your needs, as well as those of your caregiver.

Members (ages 13 and older) can use Teladoc for mental health coaching to help strengthen emotional health. The app offers tools and support for depression, substance misuse, quitting smoking and early pregnancy.

Members (ages 18 and older) enrolled in care management can get up to $600 each month (for up to 3 months) to pay for sober living at an approved facility. Members must have successfully completed inpatient substance use disorder treatment or been released from an Oklahoma Department of Corrections prison within the last 30 days.

Each household receives a $25 monthly benefit to use at CVS Health® stores. Shop for health products in store or get delivery for baby and personal care items, first aid needs and more. The oldest member in the household will receive the benefit.

If you need a ride for nonmedical reasons, we can help. We work with ModivCare to offer you 10 round-trip rides (up to 60 miles per trip) each year for things, like:

These extra nonmedical services go beyond the standard nonemergency rides benefit.
