How do you earn rewards?
You can earn rewards for completing preventive care. This is the best way to keep you and your family healthy. Everyone needs regular checkups and health screenings. They can give you peace of mind that you’re as healthy as you feel. They can also help your provider find any health problems early, when they’re easier to treat.
There is no extra cost for preventive care. Better yet, we reward you for taking steps toward health.
Earn gift cards
You can earn gift cards for each healthy step you complete. So it really does pay to check on your health. To get your reward, call us at 1-844-365-4385 (TTY: 711). You’ll want to call within 30 to 45 days of completing your screening, visit or other healthy step.
Maternity Matters® program
$25 card for enrolling in Maven and letting us know you’re pregnant (within the first trimester or within 30 days of member enrollment)
$25 card for first prenatal (before birth) visit within the first trimester
$50 card for postpartum (after birth) visit 7 to 84 days after birth
$100 card toward a portable crib or a car seat when you complete 7 prenatal visits before 37 weeks of pregnancy
Diabetes care program
$15 card for a kidney health screening (blood and urine tests required)
$15 card for a special eye exam related to diabetes
$15 card for A1C testing
$50 card each month (for up to 6 months) toward healthy food options when you complete the first three healthy steps on this list and enroll in care management (you must have a diabetes diagnosis)
Well-child visits and immunizations
$25 card for a well-child exam for kids ages 3 to 20
$60 card for completing 6 well-child visits from birth to 15 months
$40 card for completing 2 well-child visits from 16 to 30 months
$50 card for completing full series of immunizations (combo 3) before the age of 2
Behavioral health follow-up visits
- $20 card for each follow-up visit with a mental health practitioner within 30 days after an emergency mental health hospital stay (ages 6 and older)
Screenings
$20 card for a controlled blood pressure reading (139/89) with a provider (must be enrolled in care management and be diagnosed with high blood pressure as well as heart disease or kidney disease)
$25 card for completing a cervical cancer screening
$25 card for completing a breast cancer screening
$25 card for completing a colorectal cancer screening

How to get your gift card
Once you’ve completed each screening, visit or other healthy step, call us to ask for your gift card. Just be sure to call within 30 to 45 days of completing your healthy step. We’ll check the claim your provider submits for the visit to confirm you completed the steps.
Just remember, you won’t be able to use gift cards for products that can cause health risks, like alcohol, nicotine products or firearms.
