What does a care manager do?

What does a care manager do?

Care managers teach you about your health conditions and ways to take care of yourself. They also teach you about your benefits and connect you with local resources. Your care manager can:
 

  • Partner with you to create a plan that tracks your progress and helps you reach your goals

  • Help you coordinate care with your doctors or specialists and help connect you to a primary care physician (PCP)

  • Help you get rides to your doctor and drug store

  • Help you get medical and dental services, supplies and equipment

  • Support you through pregnancy services

  • Help you understand and navigate your health conditions to avoid complications

  • Get info to assist foster parents and adoptive parents
