How do you earn rewards?
You can earn rewards for completing preventive care. This is the best way to keep you and your family healthy. Everyone needs regular checkups and health screenings. They can give you peace of mind that you’re as healthy as you feel. They can also help your provider find any health problems early, when they’re easier to treat.
Earn gift cards
You can earn gift cards for each healthy step you complete (up to $50 per member). So it really does pay to check on your health.
Lead screenings
$30 for a lead screening (ages 6 months to 2 years, once a year)
Dental care
$30 for going to a dental visit (ages 0 to 20 years, once a year)
Pregnancy care
$30 for going to the first prenatal visit in the first trimester (or within 42 days of becoming a plan member) (pregnant members, once per pregnancy)

No sign up needed
Just go ahead and complete your healthy steps to start earning rewards. You’ll be signed up automatically once we receive the claim from your provider visits.
How to use your gift card
- Complete your healthy steps After you finish each healthy step and your doctor sends us the claim, we’ll mail you your gift card.
- Activate your gift card Visit My Benefits Center to activate your card online. Or call 1-888-682-2400 (TTY:711) and enter your plan member ID (some plans may ask you to enter your date of birth).
- Make purchases with your gift card Once your card is activated, you can make purchases with the funds that are loaded onto your card. You can shop online or in store at some retailers. Just remember, you won’t be able to use gift cards for products that can cause health risks. These include things like alcohol, nicotine products or firearms.
