How to sign up for Long Term Care (LTC)

The Long Term Care Program is sponsored by the Agency for Health Care Administration (AHCA). The program is offered by Aetna Better Health of Florida, a Health Maintenance Organization (HMO). This program is designed to help you continue to live in your home and community.

The State will send Medicaid recipients a letter notifying them as to whether or not they are required to enroll in the Statewide Medicaid Managed Care (SMMC) Long-term Care (LTC) program. In general, the criteria outlined below will determine whether a recipient is required to enroll or not required but may choose to enroll:

Required to enroll:

  • 65 years of age or older AND need nursing facility level of care
  • 18 years of age or older AND are eligible for Medicaid by reason of disability AND need nursing facility level of care

Not required but may choose to enroll:

  • Developmental Disabilities Waiver program
  • Traumatic Brain & Spinal Cord Injury (TBI) Waiver
  • Project AIDS Care (PAC) Waiver
  • Adult Cystic Fibrosis Waiver
  • Program of All-Inclusive Care for the Elderly (PACE)
  • Familial Dysautonomia Waver
  • Model Waiver

ACCESS Florida, the Florida Department of Children and Family Automated Community Connection to Economic Self Sufficiency (ACCESS) had links to apply for benefits or find a local service center near you.

Through our Long Term Care Program you will have your own Case Manager to help coordinate the acute, social and long-term care services you need. The Case Manager, in working with your health care providers, puts together a plan of care that best meets your care needs. When selecting a provider for the services offered by Aetna Better Health of Florida, you must choose a provider from the Aetna Better Health of Florida network.
