What rides does your plan cover?
Ambulance rides
If you have no way to get to the hospital during an emergency, call 911. We cover ambulance rides on the ground in a medical emergency for all members.
Nonemergency medical rides
We work with Modivcare to offer you rides for your nonemergency medical needs. You can get a ride for visits to your:
- Provider
- Dental provider
- Behavioral health care
There are no limits on how many rides you can use.
What rides need prior approval?
Some rides need prior authorization (PA), or approval, from your health plan before you can get them. This means Modivcare needs permission to provide certain rides.
Basic and advanced life support
Stretcher/gurney van
- Sedan, van or taxi
- Wheelchair vehicle
- Mass transit
- COVID-19 testing and vaccinations
- 1 round-trip social event for Long-Term Care (LTC) members

Ask for a ride as early as you can:
At least 1 business day before you need it for regular trips
No more than 30 days before your appointment
Sometimes you may ask for a ride with less notice, like:
Pick up after discharge from a hospital
Trips for urgent conditions (not emergencies but severe or painful enough to need treatment within 24 hours)

How to set up a ride
Be sure to ask for a ride at least one business day before you need it. Just call Modivcare at 1-866-799-4463 (TTY: 1-866-288-3133). You can call to schedule a ride Monday through Friday, 8 AM to 5 PM.
Be sure to have this info ready when you call:
- Your first and last name, date of birth and phone number
- Your member ID number
- Name, phone number and address of your provider
- Date and time of your health care appointment
- Pickup time and location (address and ZIP code)
- If anyone is traveling with you (you can take one person as an attendant, age 16 years or older)
- If you have special travel needs (example: wheelchair access)
- Pickup time for after you’re done with the appointment
Mark your calendar
Remember to mark your calendar for the date and time of your appointment. You'll want to be ready 30 minutes before your driver is due to arrive.
The Modivcare app lets you schedule non-emergency rides from your mobile device,
anytime, anywhere.
Use the app and you can:
- Schedule and change rides
- See the driver’s location in real time
- Text or call the driver for updates

Answers to common questions
Members must be 18 years or older to schedule a ride. Other people may be able to schedule a ride on your behalf, including:
Your parent
Your legal guardian
An authorized representative
Your case manager
Yes, each one-way trip cannot exceed 50 miles without prior approval.
Do you need someone to go with you on your ride? If you do, we’ll cover these ride costs for up to one person (age 16 years and older).
No, drivers are not allowed to enter a member’s home or medical facility.
If you’re not sure when you’ll be done with your appointment, contact the Ride Assist line. Just call
1-866-799-4464 (TTY: 1-866-288-3133) and you can schedule your return ride after your appointment. Make sure you have the address for where you need to be picked up. Your ride will arrive within an hour of the time you called.
Drivers are required to wait 10 minutes past the scheduled pickup time. If you’re running late for any reason, just call the Ride Assist line at 1-866-799-4464 (TTY: 1-866-288-3133).
