How to apply for MLTC
If you already have Medicaid, you can apply for an MLTC plan. Here are the steps in that process:
- Just call New York State Medicaid at 1-888-401-6582. Let them know you’d like to apply for MLTC.
- The New York Independent Assessor (NYIA) will review your info and medical care needs. They’ll make sure you qualify for MLTC.
- Once NYIA confirms that you’re eligible, you can choose Aetna Better Health® of New York as your MLTC plan.
If you also have Medicare, that’s OK. Enrolling in an MLTC plan won’t affect your Medicare benefits.

Once NYIA has confirmed that you’re eligible for MLTC, the next step is choosing Aetna Better Health® of New York as your health plan.
How to choose your plan
To choose our plan, just call us at 1-855-456-9126 (TTY: 711). We’ll connect you with an enrollment specialist to help walk you through the process. They'll answer your questions and get the process started for you.
Next, you'll talk to your assessment nurse. They’ll discuss your needs and:

Go over the provider directory with you
Talk about how to arrange your care plan

You can be approved for enrollment at any time of the month, but your coverage will start on the 1st of the following month. Once you're approved, you’ll get a welcome packet and member ID card in the mail. Then you can start using your benefits and working with your care management team.
If you’re under a provider’s care when you join, let us know. We’ll work with you and your provider to make sure you keep getting the care you need.
