Are you dealing with loneliness? You’re not the only one. Almost everyone can feel a bit lonely now and then. But if you’re feeling lonely for a long time, it can impact your health and well-being. And increase your risk of sleep problems, depression, heart problems and other health issues. You can learn more about how to deal with loneliness by finding ways to connect and getting help.

If you’re age 65 or older, you’re at greater risk of falling, which can be a threat to your health. Experiencing a fall can also reduce your ability to remain independent. Learn how to lower your chance of falling or help a loved one prevent falls.

All members have a care manager who can help with special health care needs and medical conditions. Our care managers include nurses who can:
 

  • Work with different providers, agencies and groups to help you get the care you need 
  • Help you make appointments
  • Talk about your care with your doctor
     

Care management

We help our members with chronic conditions be as healthy as they can. All members have a care manager who helps manage their care. They can help you manage chronic conditions like:

 

  • Asthma

  • Chronic obstructive pulmonary disease (COPD)

  • Congestive heart failure (CHF)

  • Coronary artery disease (CAD)

  • Diabetes

  • Electrolyte imbalance

  • High blood pressure

  • HIV

  • Sepsis

  • Urinary tract infection (UTI)

Chronic condition management

It can be hard to figure out the type of care you or your family members may need. We can help you decide whether to go to the ER or urgent care.  

 

Remember, you can use any hospital for emergency care, even if it’s not in our network. Just show your member ID card.

Mental health crisis
 

Call or text 988 to reach the 988 Suicide & Crisis Line or go to the nearest hospital if:
 

  • You’re thinking about hurting yourself or someone else
  • You have an urgent behavioral health emergency

Care management after acute care
 

Have you had a hospital stay recently? If so, you may get a call from our care management team. They’ll ask if you:
 

  • Scheduled a follow-up appointment with a PCP or specialist 
  • Were able to get any prescribed medicine 
  • Understood the discharge instructions

Flu

 

Most members 6 months and older can get a flu shot every year. Studies show which flu viruses may be the most common during the upcoming season. Seasonal flu shots protect against those three or four flu viruses. Just ask your doctor about getting the shot.

Pneumococcal
 

Two types of pneumococcal vaccines protect against a type of illness in your respiratory system (lungs and airways). Health experts recommend this shot for all adults age 65 or older.

 

COVID-19
 

Members can get the vaccine and boosters for the coronavirus (COVID-19). You can ask your doctor about getting this shot. Or check with the Centers for Disease Control and Prevention (CDC) for more info about the COVID-19 vaccine.

Substance use disorders happen when the repeated use of alcohol or drugs causes impairment. This includes:

 

  • Health problems
  • Disability
  • Failure to meet major responsibilities at work, school or home

 

These illnesses are common, serious and tend to repeat. But many people recover with treatment.  

 

If you need help with mental health or a substance use disorder, we’re here for you. We offer behavioral health treatment to our members who need it. We’ll also connect you with local resources for more support.
