Make sure to protect your eyesight with regular checkups. This gives your eye doctor the best chance to treat eye problems before they get worse. Your vision services are covered through EyeQuest. Your coverage includes:
- Visits to check your vision and eye health every 2 years
- Glasses once every 2 years

Your oral health is closely linked to your overall health. So be sure to see your dentist at least once a year. It can help prevent any problems from becoming more serious issues.
Your dental and oral health care is handled through LIBERTY Dental Plan. Your coverage includes:
- 1 exam and cleaning every 6 months
- Diagnostic X-rays
- Cavity fillings
And, if medically necessary, your coverage also includes:
- Oral surgery
- Root canals
- Crowns
- Dentures
- Implants
You will need prior authorization for these services. And to see a specialty oral health provider, you’ll just need a referral from your regular dentist.

Just call your dentist. If you don’t have one, you can search for a dentist near you.
If it’s a dental emergency, you can see any dentist. To find a dentist, just call LIBERTY Dental Plan at 1-855-225-1727 (TTY: 1-877-855-8039). If it’s a medical emergency or your life is in danger, call 911 or go to the emergency room.
Types of dental emergencies
Some examples are:
- Severe pain or infection resulting in swelling
- Uncontrollable bleeding
- Oral trauma (examples: fractured jaw, facial bones and/or dislocation of mandible)
