How do you earn rewards?
You can earn rewards when you complete preventive care. This is the best way to keep you and your family healthy. Everyone needs regular checkups and health screenings. They can give you peace of mind that you’re as healthy as you feel. They can also help your provider find any health problems early, when they’re easier to treat.
There’s no extra cost for preventive care. Better yet, we reward you for taking steps toward health.
Note: Rewards are for all plans unless otherwise noted. There may be a limit on the total amount of rewards you can receive per calendar year.
Earn gift cards
You can earn gift cards for each healthy step you complete. So it really does pay to check on your health.
Wellness visits
Diabetes well visit: $25
Complete your annual diabetes wellness exam, as recommended by your doctor. These include:
- HbA1c labs.
- Blood pressure check.
- Diabetic eye exam.
- Kidney evaluation.
Well woman exam: $25
Complete your annual well woman exam, as recommended by your doctor. These include:
- Pap smear.
- Mammogram.
- Chlamydia screening.
- Colorectal cancer screening.
- Flu vaccine.
- Pneumonia vaccine.
- Tdap vaccine.
- Shingles vaccine.
Men's health: $25
Complete important preventive care services, as recommended by your doctor. These include:
- Colorectal cancer screening.
- Prostate exam.
- Flu vaccine.
- Pneumonia vaccine.
- Tdap vaccine.
- Shingles vaccine.
Pregnancy and birth care
- $50: Earn rewards for going to your pre- and postnatal visits
Children’s health
With the Ted E. Bear, M.D. Wellness Club®, you can earn rewards for getting childhood care, such as:
- Wellness exams
- Shots (as needed)
- Weight and nutrition counseling
- Growth and development checks
Be sure to have your provider complete the incentive form (PDF) at your visit.
Well-child visits are free and include:
- A physical exam
- Shots (as needed)
- Weight and nutrition counseling
- A growth and development check
You’ll get rewards for each well-child visit:
- $10: Ages 0 through 4
- $15: Ages 5 through 10
- $20: Ages 11 through 13
- $25: Ages 14 through 17
Just for enrolling in our Ted E. Bear M.D. Wellness Club, you can earn these rewards. Just choose two from your age range:
- Ages 0 through 4: Ted E. Bear, M.D.® plush toy, portion plates, sippy cups, onesies
- Ages 5 through 10: coloring book, crayons/coloring pencils, drawstring bags, jump rope
- Ages 11 through 13: key chain/key light, water bottle, ear buds, 3-in-1 charging cable
- Ages 14 through 17: infinite phone ring, 3-in-1 charging cable, folding umbrella, USB flash drive, exercise band
If an item you choose isn’t available, we’ll send you a substitution. Just allow up to 60 days for delivery of items. Enrollment rewards are limited to one per person per qualifying year.
Young adult health
Moving On: Transitioning from Pediatrics to Primary Care: $50
Members ages 18 to 20, Complete important preventive care services, as recommended by your doctor. These include:
- Adult medical screenings.
- Weight management.
- Vaccines.
You Call the Shots
"You Call the Shots" for Teens: $50
Earn up to $50 when your teen gets these shots:
- $10 Tdap
- $10 Meningococcal
- $10 HPV (first dose)
- $10 HPV (second dose)
For completing all four shots, you become eligible for an extra $10
How to participate
To receive your rewards, you need to be a current member. You or your provider may need to complete a required form for some rewards. Visit our materials and forms page to learn about rewards for women, men and children.

Once you’ve completed each healthy step, we'll take care of the rest. Please note that certain rewards may take up to six weeks for claims processing. If you’ve recently changed your address, let us know by calling 1-800-279-1878 (TTY: 711).
Just remember, you won’t be able to use gift cards for products that can cause health risks, like alcohol, nicotine products or firearms.
Members are eligible for one incentive per calendar year per reward type.
