How do you earn rewards?
You can earn rewards when you complete preventive care. This is the best way to keep you and your family healthy. Everyone needs regular checkups and health screenings. They can give you peace of mind that you’re as healthy as you feel. They can also help your provider find any health problems early, when they’re easier to treat.
There’s no extra cost for preventive care. Better yet, we reward you for taking steps toward health.
Note: Rewards are for all plans unless otherwise noted. There may be a limit on the total amount of rewards you can receive per calendar year.
Earn gift cards
You can earn gift cards for each healthy step you complete. So it really does pay to check on your health.
Wellness visits
Complete your annual diabetes wellness exam, as recommended by your doctor. These include:
- HbA1c labs (HbA1c < 8): $15
- Blood pressure check (less than 140/90): $15
- Diabetic eye exam: $10
- Kidney evaluation: $10.

Womens health
Complete your annual well woman exam, as recommended by your doctor. These include:
- Pap smear: $10
- Mammogram: $10
- Colorectal cancer screening: $15
- Flu vaccine: $10
- Preventive care: $30

Men's health
Complete important preventive care services, as recommended by your doctor. These include the following:
- Preventive care: $30
- Colorectal cancer screening: $15
- Flu vaccine: $10
Have your provider complete the incentive form (PDF) at your visit. This form is also available in Spanish (PDF).
Pregnancy and birth care
- Earn one $50 gift card for going to your prenatal appointment within the first to 15 weeks of pregnancy.
- Earn one $25 gift card for your postpartum checkup, which you should have 1 to 12 weeks (7 to 84 days) following the birth of your baby.

Children’s health
With the Ted E. Bear, M.D. Wellness Club®, you can earn rewards for getting childhood care, such as:
- Wellness exams
- Shots (as needed)
- Weight and nutrition counseling
- Growth and development checks
Be sure to have your provider complete the incentive form (PDF) at your visit. This form is also available in Spanish (PDF).
You’ll get rewards for each well-child visit:
- $10: Ages 0 through 4
- $15: Ages 5 through 10
- $20: Ages 11 through 13
- $25: Ages 14 through 17
You Call the Shots
- HPV vaccine: $50 (complete two doses by the age of 13)
- Flu series: $50 (children under two years of age, complete two doses)
For completing all four shots, you become eligible for an extra $10

Behavioral Health Follow Up Care
Seeing your primary care provider (PCP) after you leave the hospital can help you feel better and stay on track. If you have a visit with your PCP or a specialist within 7 days or 30 days after discharge, you may earn a reward.
- 7-day follow-up after behavioral health hospitalization: $25 (two times per 12 months)
- 30-day follow up after behavioral health discharge: $25 (three times per 12 months)
How to participate
To receive your rewards, you need to be a current member. You or your provider may need to complete a required form for some rewards. Visit our materials and forms page to learn about rewards for women, men and children.

Delivery and rules
Once you’ve completed each healthy step, we'll take care of the rest. Please note that certain rewards may take up to six weeks for claims processing. If you’ve recently changed your address, let us know by calling 1-800-279-1878 (TTY: 711).
Just remember, you won’t be able to use gift cards for products that can cause health risks, like alcohol, nicotine products or firearms.
Members are eligible for one incentive per calendar year per reward type.
You must be an Aetna Better Health of Virginia member at the time of service to quality for a wellness reward.
