Need a ride to your health care visits? Let us help you get there. You can get rides to Medicaid services we cover — at no cost to you. Just contact us to learn more.

Ambulance rides

 

If you have no way to get to the hospital during a life-threatening emergency, call 911. We cover ambulance rides on the ground in a medical emergency.

 

You can also use any hospital for emergency care, even if it isn’t in our network. Just show your member ID card. 

 

Nonemergency medical rides

 

We work with ModivCare for your nonemergency medical needs. You can get a ride for visits to your:

 

  • Provider
  • Dental provider
  • Pharmacy
  • Behavioral health care

You can use as many nonemergency medical rides as you need (up to 50 miles round trip).

 

Nonmedical rides to local resources and services

 

Need a ride to the grocery store or food bank? The local library or place of worship? You can get rides to other resources and services, too. And it doesn’t need to be for any medical reason. Your covered rides include 30 round trips or 60 one-way trips each year.

When to ask for a ride

Ask for a ride as early as you can, and at least three business days before you need it. 

 

Sometimes you may ask for a ride with less notice, like:  

 

  • Pickup after discharge from a hospital 
  • Trips to the pharmacy for medication or approved medical supplies 
  • Trips for urgent conditions (not emergencies, but severe or painful enough to need treatment within 24 hours)
  • Trips for getting life-sustaining treatment or any treatment to help prolong your life, like surgery

Need to cancel your health care visit? You’ll want to let us know before the approved and scheduled trip.

Members ages 18 years or older can schedule a ride. Remember to ask for a ride at least three business days before you need it.

 

Just call ModivCare (formerly LogistiCare) directly at 1-800-734-0430 (TTY: 711). Or schedule your ride online. They’re here for you 24 hours a day, 7 days a week.

Be sure to have this info ready when you call:

 

  • Your first and last names
  • Your date of birth
  • Your home address, including ZIP code, for the pickup
  • Your phone number
  • Name of your provider
  • Date and time of your health care appointment 
  • If anyone is traveling with you (you can take one adult and one child)
  • If you have special travel needs, like wheelchair access
  • If you have special health care needs

 

Get the Modivcare app

 

The Modivcare app lets you schedule non-emergency rides from your mobile device, anytime, anywhere.

 

Use the app and you can:

 

  • Schedule and change rides
  • See the driver’s location in real time
  • Text or call the driver for updates

How to download


Just search for Modivcare app from the Apple® App Store® or the Google Play™ store. Then, download. You’ll need to have your email address handy.

Ride tips to remember

 

 

Know who you can bring

 

Do you need to bring someone with you while you travel? If yes, we’ll cover the ride costs for up to:

 

  • One other adult
  • One child

Mark your calendar 

 

Remember to mark your calendar for the date and time of your appointment. You'll want to be ready 30 minutes before your driver is due to arrive. Your driver will also contact you soon before they arrive.

 

Be aware of travel requirements for children 

 

  • Aged 14 and younger: Must have a parent, guardian or other authorized adult with them
     
  • Aged 15 to 17: Must have a parent, guardian or other authorized adult with them or have consent from one of these on file to travel alone
     
  • Aged 7 and younger: Must be in a booster or child safety seat while traveling in the vehicle
     
  • No need for parental consent if the health care service is confidential in nature

Are you a member in a DD waiver program? If so, you can get covered rides for traveling to and from your non-waiver services. The Department of Medical Assistance (DMAS) Transportation provides coverage for these rides. 

 

To schedule a ride, just call 1-866-386-8331 (TTY: 711). You can get help Monday through Friday, 6 AM to 8 PM. You can also visit the DMAS Transportation website to learn more.

 

Have an urgent need? Or problems with the service?

 

Just contact us. Then, ask to speak with a care manager. They’ll work with you and your DD or Intellectual disability (ID) waiver case manager to help you get the services you need.
